Membership Invitation

This form may be utilized if you wish to have an individual invited to membership in our organization.  Simply provide the information requested below and we will issue an invitation in your name.  If for some reason you do not wish to have your name used, be sure to indicate this in the comments section of the form.  The data line "Sponsor" is for your name.

Please remember also that the Board of Directors authorized a full year's additional membership in the organization for any current member who finds 5 new members in any given year.  These members must be confirmed by payment of membership fees.  The treasurer must be apprised of the sponsor at the time the membership is accepted.

Once you submit the data electronically, you will immediately receive an acknowledgement.  The invitation will be issued as soon as possible.  If an e-mail address is provided, it will be utilized.  If not, the invitation will be sent by regular mail.
